ClearVista Healthcare is paying close attention to all reports and recommendations surrounding the COVID-19 virus. We place the health and well-being of our patients and staff at the forefront of all we do.

Another week goes by of our new normal. I want to acknowledge our staff and employees who continue to focus on providing the very best patient-centered care despite the challenging times. I also want to acknowledge many of you, who despite the physical barriers, have found creative ways to stay in touch with your loved ones. While visitation restrictions need to stay in place, we are committed to working with you on connecting with your loved one, whether that be on FaceTime, Skype, Facebook, or even through a window.

 

Please continue to trust we are centering all of our efforts on the care and well-being of your loved ones:

• We continue to monitor our residents on an every shift basis.

• We continue to monitor our staff daily. In addition they are now wearing a face mask during the entirety of their shift.

• We continue to participate on national, state and local work groups to be informed of all recommendations directions and mandates and adjust our policies and procedures routinely to reflect current best practices.

 

We have been humbled by the amazing acts of kindnesses that families have done from sending meals in for our entire staffs, sending flowers and balloons, posting messages on windows and front doors thanking us for the care that we are providing. We thank you for generosity and know it is appreciated.

 

We continue to be sensitive to the restrictions of your ability to visit and see your loved ones, we have been committed to sharing stories on social media on a daily basis. We are being compliant to ensure we have consents and permission prior to posts. We encourage you to join us on social media platforms such as Facebook, Twitter and Instagram to see some of the amazing smiling faces of our residents and employees on a daily basis. SERVICES

ClearVista cares for a broad range of emotional needs while specializing in the care of adults and seniors and the many co-occurring medical conditions that can accompany aging.


Comprehensive Psychiatric Assessment

Your loved one will be assessed by a psychiatrist that specializes in understanding the treatment needs of our senior population. Current emotional, functional and medication needs will be evaluated and addressed during the inpatient stay.


Individualized Treatment Plan

Our team will work with you and your loved one to establish a plan of care that is both comprehensive and effective. We do this on a one on one basis, as well as in a treatment team setting. After our initial assessment, we will establish both short and long term goals that support both the ongoing behavioral health needs and the individual patient and family preferences and goals.


24-Hour Nursing Care for Assessment Monitoring

Registered Nurses are available 24/7 to provide support, education and structure to the unit population, monitor the medication and care needs of your loved one, and supervise treatment programming. An RN is available for questions in person during all visitation times and via the phone after hours.


Medication Management & Education

Our medical staff includes both psychiatry and internal medicine physicians with geriatric specialty training. They work together to provide for the physical and behavioral needs of your loved one. After conducting a comprehensive assessment, the best medication to treat the presenting symptoms will be prescribed. Education and monitoring related to medications is provided by our physicians, on site pharmacist and RN’s.


Symptom Management

Assessments are conducted three times a day to ensure that symptoms and behaviors are managed with the most current information and stats.


Nutritional Consultation

ClearVista Health and Wellness provides three nutritious meals a day as well as 3 scheduled snacks. A registered dietitian will evaluate special dietary restrictions and needs to ensure ongoing nutritional support.


Therapeutic Interventions that Promote Stress Reduction

Over 5 hours of therapeutic treatment and recreation therapy is scheduled daily. Our team focuses on increasing coping skills, reducing behaviors that limit socialization and exercise and recreation therapy to maintain the highest level of patient functioning.


Exercise Therapy

Groups are conducted to increase physical endurance and maintain active involvement while in our hospital. Weather permitting, we utilize our handicap accessible outdoor courtyard for activities and exercise.


Expressive Therapy

Music, art therapy and other forms of expressive therapy are conducted on a biweekly basis. Expressive therapy helps to engage your loved ones in memories and feeling that support their journey to recovery.


Family Participation

We value and encourage feedback and participation from families, and other supportive individuals, to maintain social connections and supports


Discharge Planning

Our care coordinators work with family preferences and referring facilities to ensure a comprehensive and supportive discharge plan. Planning takes into consideration patient and family preferences as well as the treatment needs of the individual. For more information, ask to speak to one of our care coordinators.

Family Visitation

We always welcome family members of senior guests from 2-3 p.m. and 6-7 p.m. and family members for adult guests from 5:30-6:30 p.m. daily to provide support and meet with our inpatient clinical team members. The visitation is limited so our team can focus on the treatment needs of your loved ones.

SteppingStone at ClearVista is our outpatient program designed to promote the self-reliance and confidence needed for individuals to integrate back into the community.

Outpatient

SteppingStone at ClearVista is our outpatient program designed to promote the self-reliance and confidence needed for individuals to successfully re-integrate back into the community. We offer the appropriate follow-up support to continue and reinforce the life skills and clinical therapies learned during their stay at ClearVista.

We also offer treatment and therapy to individuals that may not need hospitalization, but who will benefit from the structured programming on a daily basis.

How to make a referral

1. Call referral line: 440.960.7963 between the hours of 6 a.m. and 10 p.m. or 440.960.7960 outside of those hours.

 

2. Fax patient information: 440.960.7990

 

3. Once the information is received, our staff will verify insurance and contact our Medical Director.

 

4. The Medical Director will be presented with the information and will determine if admission to our setting is appropriate and meets criteria.

 

5. If admission is appropriate, we may ask that patient is cleared through area Emergency Department, depending on patient needs.

 

Not appropriate for admissions

Acute medical or surgical illnesses that may prevent participation in active treatment programs.
